Monique Powers Beauty Academy

I certify that:

1) I have a basic understanding and knowledge of the lash lift and tint process

2) I am at least 18 years of age or older


3) I am a licensed cosmetologist, esthetician or barber OR a currently enrolled student at a reputable beauty school or accredited beauty program


4) I have been previously trained to do a lash lift service by Monique Powers and/or another reputable brand.

5) I am allowed to perform this service in my city and/or state


6) Performing a lash lift service is within the scope of my state issued or soon to be issued license.

I UNDERSTAND AND AGREE THAT ALL SALES ARE FINAL. ONCE I PAY FOR THE LASH LIFT AND TINT MASTERCLASS, THERE WILL BE NO PARTIAL REFUNDS, FULL REFUNDS, OR CREDITS GIVEN OF ANY KIND OR AT ANY TIME.
